As you or your loved one progress through the recovery process and nears the end of treatment, the
JNC has a dedicated Discharge Case Manager that will begin working with you in week 2 to make sure
there is a solid plan in place by the time the residential program is completed. The Discharge Case
Manager will help you develop your plan for your next steps in your recovery journey. This is called
discharge planning, and it may include making connections in the community that can help promote
long-term success. We help the client with things like getting scheduled for their next level of services,
helping link clients with other services that may be offered in the community, applying for sober living,
etc. For clients that may be moving back to their home outside of Brown County we will help identify
service providers, support groups and therapists, and will assist with special living arrangements, if
needed.
